Food Sampling Form

Complete this form ONLY if you will be giving out/providing open food samples from your booth.

The Orange County Health Department will be performing inspections of all booths serving food samples at the show.  All exhibitors who are serving food or beverage samples from their booth are required to submit this form. 

The following is required:

  1. Submit this form by September 23, which will be provided to the Orange County Health Department
  2. Ensure you are prepared to be in compliance with all On-site Food Sampling and Handling requirements related to preparing and distributing samples
  3. Reference the California Retail Food Code guidelines as a guide to what health department inspectors will be evaluating as they inspect booths

Are any samples or food that are being provided PHF (perishable)? *
Will you be cutting, preparing, or cooking any food products in your booth? *
Will Aramark be preparing or cooking your food products? *
Will you be using multi-use utensils? *
Are any of your food items imported into the US? *
If food items have been imported, has the product already been registered with the FDA? *

Terms of Agreement

Exhibitor understands that the Orange County health inspectors shall be present at the show and inspecting all booths serving food samples to ensure they are in compliance with Food Safety/Sampling guidelines and the California Retail Food Code guidelines. *
Exhibitor understands that Produce Marketing Association will be providing all information included in this application to the Orange County Health Department. *
Exhibitor agrees that Produce Marketing Association is not liable for any actions of the Orange County Health Department or costs incurred by exhibitor in order to be in compliance with guidelines. *
