Submit one form per musician. Complete a separate form for each additional absence.   
Musicians and Parents/Guardians are required to submit an Absence Form to report any scheduled absences, late arrivals, or early departures, at least 2 weeks in advance. A complete Absence Form must give a detailed reason for the absence.

POSITIVE COVID-19 ABSENCE - SUBMISSION OF PROOF REQUIREMENTS
When submitting a photo of your positive COVID-19 test result, the photo must include your full name and an electronic or analog record of the date and time your test was taken.

COVID+test
"CANCEL PREVIOUSLY REPORTED ABSENCE" means you intend to be at rehearsal. Marking this selection means you have previously reported an absence that you are now canceling.
EARLY Departure Procedure: Exit quietly and gracefully. IF your schedule changes and results in your staying the entire rehearsal time, please let the attendance coordinator know when you check-in, unless you have already submitted a cancellation request in the system.
 
CAUTION: Three late arrivals or early departures will count as an absence, which counts towards your four allowed absences.

*IMPORTANT NOTICE FOR WS, AO & SO WIND AND BRASS PLAYERS
The orchestra must have a full wind and brass section at rehearsals as much as possible.
Please ensure a member of your section is able to cover your part for rehearsal so we always have a full complement of wind and brass players in each piece. Notify your Section Leader of your absence.

Please Notify your stand partner of your absence so that they remember to bring their music to rehearsal and make necessary adjustments to the seating arrangements at rehearsal. (Note, every string player should always bring their music to rehearsal.)

Please Notify Mr. Artie Storch and your section of your absence so that necessary adjustments can be made at rehearsal to cover essential parts.
 
As a general rule, and why all percussionists have a set of all the parts to each piece, the TIMPANI part should always be covered.
