IMPORTANT
Before Starting – there are FOUR THINGS YOU WILL NEED.
1. OSIS Number/Student ID: Usually a 9 digit number attached to the student.
2. Parental Consent: Parent or guardian must approve this application(Photo ID REQUIRED)
3. Doctors Note: The doctor's note must clearly state the student is cleared for employment or physically fit for work. - General doctor notes WILL NOT be sufficient.
4. Student Email Address: This DOES NOT have to be a Midwood email address. Just an email address that the student has access to. We send notifications to both student and guardians.
Please understand your working papers will not be approved with out these items being submitted accurately.
