PLEASE NOTE: THIS FORM IS ONLY FOR CDP DELEGATES AND GUESTS WITH DISABILITIES REQUIRING ACCESSIBLE FEATURES IN THEIR HOTEL ROOM.

Delegates and guests not requesting accessibility features in their hotel room or are only looking to request a room closer to the elevator please DO NOT use this form. Please make a reservation directly with the hotel via the links on the Convention website and then follow up with the hotel if wanting a room closer to an elevator.

All hotel room requests will be taken on a first come, first served basis.

Please note, this is a room request form only. Reservation confirmation numbers will be emailed directly to guests by the hotel no later than 72 hours after October 30, 2023.

If for any reason your request(s) cannot be fulfilled, you will be contacted promptly by a CDP Housing Team member.

The submission deadline for all HOUSING REQUEST FORMS, serving participants with disabilities is October 23, 2023.
