2008-2009 Wichita Falls Wildcats Jr. A Tryout Registration Form

Dear Wildcats Prospect,
The 2008-2009 Wichita Falls Wildcats will be holding tryouts June 6,7, & 8, 2008 at Munn Ice Arena (Michigan State University) in East Lansing, Michigan.
The Wildcats are looking for 23 individuals who possess a strong work ethic and the goal to make it to the next level of hockey.
We ask that you complete the questionnaire as soon as possible. The cost of the camp is $250.00 and includes USA Hockey Insurance, a jersey, one skills session, three games (minimum), and an All Star Game for qualifying players. Due to the goaltender selection process, goalies may be the last confirmed positions.
If you are accepted for the camp, you will receive a confirmation letter that includes registration times, team information (practice/games), hotel information, and directions. It is your responsibility to carry personal medical insurance; the Wichita Falls Wildcats cannot be held responsible for any incident that occurs during camp. Last minute changes may occur to your team and schedule.
