This questionnaire is intended to be a guide to help you think about all the ways a pet will impact your life and also determine if a pet is right for you. All of the questions concern various aspects of pet ownership that should be given serious consideration before deciding to share your life with any animal.
1) If for any reason, at anytime during the animal’s life, I am unable or unwilling to care for them, I agree to return the animal to Brooklyn Animal Foster Network at my own expense no matter where I reside. I will not give the animal to anyone else.
2) I agree to allow a Brooklyn Animal Foster Network representative to visit my home to do a home visit before and or after I adopt the animal.
3) I agree to provide the animal with proper food, shelter and water at all times, as well as yearly vet checkups and shots and additional medical care if the need arises.
4) I agree to license my animal as required by law.
5) I agree not to put the animal to sleep, unless in case of terminal illness or suffering, without the permission of Brooklyn Animal Foster Network.
6) I agree that I will never declaw my pet.
7) I will notify Brooklyn Animal Foster Network promptly of any change of address or telephone number.
8) I will notify Brooklyn Animal Foster Network immediately if my animal ever gets lost or stolen and will make every effort to recover the animal.
9) I agree to never allow this animal to be used for the purposes of vivisection, experimentation, the purposes of entertainment, or any other purpose except as a house pet and companion.
I fully understand the above conditions and agree to abide by them. I understand if I break this contract in any way my animal may be taken away from me by Brooklyn Animal Foster Network and I will be prosecuted to the full extent of the law. I restate my understanding of my obligation by initialing here to return my adopted animal to Brooklyn Animal Foster Network at anytime during the animal’s life if I am unwilling or unable to care for the animal.
In acquiring this pet from BAFN, I AGREE THAT: I will procure a veterinary examination within one week of adoption and I will assume all costs for preventative care and illness or injury for this animal. If it is not already neutered, I will have the pet neutered within 30 days of adoption or as soon as my veterinarian is willing to perform the surgery. I will include this pet as a member of my household and provide it with proper care and treatment.
I will obey all state and local ordinances and statutes on animal care and regulations. No animal with a known medical condition requiring special care and or expense is released for adoption without giving the adopter all available information about the condition and care of the pet.
Should your pet exhibit symptoms of illness, contact your veterinarian and advise BAFN if your veterinarian feels you should return the animal.
I agree that the animal I have adopted from BAFN is NOT to be sold for any reason to another person or agency. I agree to contact BAFN if for any reason I become unable or unwilling to keep and care for the animal.
I understand that the adoption fee is not refundable unless the pet has been deemed by a licensed veterinarian to have been medically unsuitable for adoption at the time of placement.
In signing this adoption agreement with BAFN, I declare that I am aware that: animals are different from human beings in their responses to human actions; the actions of animals are often unpredictable; children and animals must be closely supervised when interacting; an animals’s behavior may change after it leaves the shelter and accustoms itself to its new environment.
I am aware that the educated opinions of the shelter staff do not constitute a claim or guarantee as to the temperament, health or mental disposition of any animal available for foster or adoption. I herby accept possession, legal guardianship and responsibility for the animal I am adopting and discharge BAFN forever from liability for any injury or damage to any person or property caused in the future by said animal. and from any causes of action, claims, suits, or demands whatsoever that may arise as a result of such injury or damages I hereby accept possession, legal ownership and responsibility for the animal

Although your animal is up to date on its vaccinations and wormings, many (especially the young) will require further vaccinations and follow-up wormings.
